I think the second option is the better one. I would update the plan each year and add another year to it.
Regarding naming the thing, naming schemes in general are an intersting topic. I like to use a 'theme' that fits the purpose of the thing to name. I'll give you an example for what I mean.
There is a book about starting a business called "The Art of the Start" or so by G. Kawasaki. The name of the book is such a theme. It is a derivative of the most famous book on warfare, called "The Art of War". It's from a Chinese general called Szun Tun. In his book Kawasaki names a couple of things after items from asian culture, sticking to this "Art of War"-theme. For instance he talks about a business plan called a "Mat" which is somehting from asia I forgot what it was, sorry. Or he uses the asian "mantra" instead of a western word like "motto" and so on. I think it became clear what I mean with "theme".
Now, I said I like to use themes that support the purpose of the thing to name. Kawasaki did the same! At least I would argue that a warfare analogy is a good fit for starting a business and establishing it on the market. It has the same air of "engagement" and "fight".
So, what would be a good theme for a personal 5 years plan?
It depends on your outlook on life for one.

Maybe you could answer her question and after that you could find a "theme" that matches that and then find a set of names according to that theme?
It's up to you I guess. In any case you can see that I am way to geeky to just name something a "plan" and be done with it.
