Notepad with three 3x5 perforated 'pieces' per sheet?

DA mentions these in one of his Audiobooks - I think in 'Getting Things Done Fast'.

I've looked but cannot find these. Where can I get them? He describes a standard notepad that has three perforated 'pieces' per sheet. You can write a small note on one part of the page, tear it off, and use the rest of the sheet for more notes.
