Calendar apps only notify, they don't alarm. What do you use for time-critical events?

I use a calendar app for events such as meetings, calls, and appointments - also known as hard landscape items that need to be attended at a specific time. But the calendar apps only provide sound notifications that ring once at a specific time, and this is easy to miss. And it's easy to miss because when I'm focused on some work, I'm not looking at the calendar, so I need an alarm to remind me that an event is coming up.

The clock app has alarms, but that requires manually creating the alarms for calendar events each day, so it's not efficient, although it's effective. I use a GTD-based app for untimed next actions and a calendar app for time-specific commitments. And I'd rather not add a third app because it either duplicates what's in the calendar or become another place I have to check and keep in sync. So I am wondering if there is something simple that others here are doing that resolves this issue.

What do you use to remind you of events that you need to attend at a specific time?
