Successful outcome of advising

Working as an occupational safety engineer at a public hospital in Germany, one of my job duties is advising the supervisors and managers on occupational safety and health topics and related problems. The advising is often done after we have inspected the workplace in question and written a report about the inspection. Then the manager who is responsible for the health of the employees working at that workplace or workshop is given advice on how to solve the problems.

Being new to the GTD method I have a question about projects:
What would be in the GTD context the successful outcome (physical outcome) of a project that consist of the steps "inspection, report, advice"?

Well, I would probably state it as "hospital Thus And So successfully advised", that being the final step which denotes that you are done.

Suppose there is a problem that has to be fixed. Are you responsible for the problem being fixed, or just for informing the department that there is a problem and how to fix it?

If the safe practices of the hospital are your responsibility, then the outcome should be that the problem has been fixed. For example, "department X will label medications according to protocol Y," or "department Z will have zero incidents per year of (something bad)."

If you're only responsible for informing the department that they have a problem, and telling them how to fix it, then it should be "inspect department X and make sure that they understand their responsibilities."

I'm just guessing. I don't work in the health professions, though I am (unfortunately) somewhat familiar with hospitals and protocols.

I think I would look at it as the completed inspection. If I understand your process correctly, the key activity is the inspection and the reporting and advising are aspects of the inspection. Essentially, you can't report or give advice without completing an inspection. So you may finish inspecting a facility (the physical activity), but the project "Complete inspection of facility Z" isn't complete until you complete the report and issue your advice.

I would list the inspection site or source as the project.
The next steps would be whatever was required to complete the inspection
ie:
Confirm inspection date @ Calls
Inspect ....... @ Site(location etc...)
Review Notes @ Office
Organize notes for Report @ Office
----------------------------------------------------------------
I would then list report as a sub project or parent
Draft report @ Computer
Edit report @ Computer
Complete final draft @Computer
Copy Final report for distribution @ Office
-----------------------------------------------------------------
Advising would be another sub heading
Contact nesseary individuals @Call
Confirm meeting date @Calls
Prep reports for distribution @ office
---------------------------------------------------------------
For the actual meeting I would list the highlights of the report like.
Point out door hazard @Agenda
Elaborate on the lack of first aid apparatus @Agenda
So forth and so on
